Payment and Insurance Information
Initial Consult - Please arrive 15 minutes prior to your appointment. Bring your medications and any current insurance cards that you would like for our office to file claims for you.
Insurance - Our office accepts Medicare and most commercial insurances. We will file insurance claims to the insurance companies that you provide current information on at the time of service.
Payment - Copays and Coinsurance are due at the time of services. Any remaining balances due to noncoverage of deductible or other reasons are due at time of billing.
Precertification - If your insurance requires precertification for visit let us know prior to appoint. We will get precertification for any procedures we order at time of scheduling.
